• Pearl 25 HMO offers members no deductible on most services and a low or no copay.  Services include prescription drugs with a $100 deductible, preventative services, office visits, and hospitalization.  Members choose a primary care physician who coordinates members’ healthcare needs.  The primary care physician will refer members to specialists within the network if necessary.

• Diamond 15 is a PPO plan.  PPOs allow members to see non-network healthcare providers as well as network providers.  Members do not need a referral to see specialists from either within or outside of the network.  Diamond 15 has a choice of premiums ranging from $250 to $7,500 for individuals.  Coinsurance, after meeting the deductible, is generally 20% for network providers and 50% for non-network providers.  Services covered include doctor visits and hospitalization.  Prescription drugs are available with a separate deductible.

• Emerald 40 is a PPO similar to Diamond 15 in services.  Coinsurance for network providers is 30% and non-network providers, 50% after meeting the chosen deductible. Deductibles range from $1,000 to $10,000 for individuals.

• Garnet 50% is a PPO plan with a choice of no deductible or a $10,000 deductible for individuals.  Members pay 50% coinsurance to both network and non-network healthcare providers after their deductibles have been met. 

• Crystal HDHP is an HSA plan using a PPO.  HSA plans may save money on their income taxes by contributing pre-tax dollars to a special fund from which they will pay qualified medical expenses.  These accounts can accrue interest and any funds left over at the end of the year can be rolled into the next year.  HSA plans offer deductibles for individuals ranging from $1,000 to $3,500 for network providers and $2,000 to $7,000 correspondingly for non-network providers.  After meeting deductibles, members have a 20% coinsurance for network providers and 50% for non-network healthcare providers.  Services included are office visits, hospitalization, and prescription drugs. Prescription drugs have a separate deductible.
