• Heritage Protector Plus 20: allows members to see healthcare providers from both within and from outside of their network, however, members will enjoy more savings by using network providers.  Members have a choice of two deductibles.  Coinsurance is 20% when seeing network providers and 50% when using providers who do not belong to the network.  Services include preventative care exams, diagnostic services, and hospital services.


• Heritage Value Plus 30: offers a choice of three deductibles and a 30% coinsurance when seeing medical providers who belong to the network and a 50% coinsurance when seeing non-network providers.  Coverage includes prescription drugs with a separate deductible, diagnostic services, and hospitalization.
