Excellus Blue Cross Blue Shield of New York

While Excellus does offer traditional insurance options, they are more well-known as providers of state-funded aid programs. Some of the many plans they currently offer to low and middle-income residents of the state include:

• Child Health Plus (CHP): This is a state-sponsored plan, and provides children under 19 who live in New York state with quality insurance. Premium amounts are determined by the household's gross income per month, but all children are eligible regardless of family income.
• Healthy New York: This plan serves those who are living with incomes below 250% of the federal poverty limit. For hospitalization, a $500 co-pay is in effect, while emergency room visits cost $50 (unless admitted, then the cost is waived). Office visits are also covered with a small co-pay.
• Blue Choice Personal HMO and POS: These plans allow members to choose care from in or out of the network. Care received in the network involves paying a co-pay for service, whereas out of the network, members pay a co-insurance. For physician visits, consumers pay 20% coinsurance or a $200 copayment, whichever is less.
• Blue Choice Option HMO: This is directed for those who are on Medicaid. For no cost, members receive laboratory services, OB/GYN care, and medical care from a PCP. There are income limitations with this program.
• ValuMed/ValuMed Plus: For consumers who make too much to qualify for Medicaid, this policy covers inpatient and outpatient care, maternity care and emergency room visits. It is open to those individuals who make up to 250% of the federal poverty limit.
• Family Health Plus: This plan is for adults who are limited financially but are not eligible for Medicaid. It is offered through cooperation with the New York Department of Health, and includes comprehensive medical coverage, with hospitalization, lab tests/x-rays and vision services included at little or no cost.
