Some of the many plans they currently offer to low and middle-income residents of the state include:
- Direct Pay HMO/POS: This plan is a cross between a network and non-network plan. Full coverage is offered for services that are received in-network, while those provided out-of-network are paid for on a point-of-service basis, with a percentage co-insurance and co-pay amount.
- Child Health Plus (CHP): This is a state-sponsored plan, and provides children under 19 who live in New York state with quality insurance. Premium costs are determined by the household's gross income per month, but all children are eligible regardless of family income.
- Healthy New York: This is similar to the Family Health Plan, but is designed to meet the needs of those with incomes below the federal poverty limit. For hospitalization, a $500 co-pay is in effect, while emergency room visits cost $50 (this cost is waived if the patient gets admitted to the hospital). Office visits are also covered with a small copay.
- Traditional Plus: This is a hospitalization plan, and covers both in and out-patient hospital benefits, including semi-private boarding, laboratory work and surgical procedures.
n addition, Empire BCBS offers dental and vision plans, as well as access to health and wellness plans to fit the budgets of many different families.
