Some of the features of the direct-pay BCBS of Arizona health plans for individuals and families include:
- Separate rate scales for in-network and out-of-network, but flexibility to use doctors and facilities near you.
- After the deductible is met, you pay 20% of all costs.
- No maternity care except for complications.
- Non-deductible for prescription drug co-pays on most plans.
- Preventive care and immunizations are offered with the deductible waived.
Individual plans include a $250 deductible, $500 deductible, $1,000 deductible, $1,500 deductible, and a $2,500 deductible plan. BCBS of Arizona also offers a high deductible plan for use with a health savings account and a short-term coverage plan. All plans are limited to residents of Arizona who are not eligible for Medicare; however, some out-of-state procedures are covered.
