- Separate rate scales for in-network and out-of-network. For instance, an in-network hospital stay would have no deductible and a co-pay of $200 per day for the first five days, while covered inpatient expenses would be covered 100% by the PPO allowance. An out-of–network facility would have a $2,000 deductible, while inpatient expenses would be paid at 50%.
- Outpatient surgery is covered at in-network facilities at 100% after a co-pay of $300.
- In-network doctor visits are covered at 80% after the yearly deductible is met.
Other individual plans include a $1,000 deductible plan and a $2,500 deductible plan. BCBS of Alabama also offers a high deductible plan for use with a health savings account and a short-term coverage plan. All plans are limited to residents of Alabama.
