Arizona United Healthcare provides plans that allow members to make their own choices about their health needs. For example, members can visit physicians and specialists without a referral in the open access plans.
Arizona United HealthCare provides extensive provider networks, competitive pricing, comprehensive products, and excellent customer service. With the variety of individual plans and group plans available, Arizona United HealthCare ensures that its customers are well serviced by the option most suitable to their needs.

Individual and family plans are provided through Golden Rule Insurance Company, a subsidiary of United Healthcare. Plans include:

  • Copay Plans
  • Health Savings Account Plans (HSAs)
  • High Deductible Plans

Employers can choose from a variety of plans that offer different benefits to employees:

  • Choice and Choice Plus – members can visit any provider in the network, with no referral necessary. Choice Plus also includes non-network coverage.
  • Definity Health Reimbursement Accounts (HRA) – allows employers to share the costs with employees.
  • Definity Health Savings Account (HSA) – savings for cost-effective services and 100% coverage of in-network preventive care.
  • Managed Indemnity – similar to traditional heath insurance, but once the deductible is paid, a percentage of medical expenses are covered; referrals are not necessary.
  • Options PPO – access to large network of physicians, direct access to specialists.
  • UnitedHealth Basics – affordable coverage for part-time and hourly employees.
  • Select and Select Plus – members utilize a primary care physician from the networks, requiring referrals to specialists.
