Specifically, Affinity Health Plans include coverage for families and seniors through the following programs:

• Child Health Plus – designed for uninsured children, 19 or younger, who do not qualify for Medicare or Medicaid.
• Family Health Plus – for uninsured, low-income residents who do not qualify for Medicaid or Medicare.
• Medicare Advantage – for residents in the New York City area who are eligible for Medicare and government payment of Medicare cost-sharing.
• Medicaid Managed Care – for Medicaid beneficiaries who are eligible through the Temporary Assistance to Needy Families, Transitional Medical Assistance, or Supplemental Security Income programs.
• Family Planning Benefit – available to Child Health Plus and Family Health Plus Members.
• UniCare – uninsured individuals and families already in an Affinity Health Plan program may elect this option; a portion of the premium is paid by the State, with the remaining premium and co-payments paid by the member.
